Quebec voluntary disclosures: New prescribed form

Revenu Québec has updated its voluntary disclosures application form.

By Revenu Québec’s count, an average of 500 taxpayers rectify their tax situation under its Voluntary Disclosure program each year. 

Now, Revenu Québec has released a new form for taxpayers and their advisers making applications under this program. New Form LM-15-V, Voluntary Disclosure Application, can be used for applications to correct tax filing errors and omissions concerning either Quebec income tax or GST/HST.

The new form must be used for Voluntary Disclosure applications made on or after September 19, 2013.
